Care Coordination Study - Hawaiʻi

Are you receiving treatment for breast cancer?  We need your help to improve cancer care!

We would like to invite you to participate in our study!

In this study you will be asked to:
– Complete a 15–20 minute survey about care coordination experience
– Return the survey in a provided, pre-paid postage envelope

ELIGIBILITY:
• 18 years or older
• Reside in Hawaiʻi
• Have started your first treatment for invasive breast cancer after 01/01/2023
• Have received treatment for invasive breast cancer for at least 2 months
