Care Coordination Study - Maui

Are you a cancer patient?  We need your help to improve cancer care!

We would like to invite you to participate in our study!

In this study you will:
– Watch educational videos about cancer, care coordination & self advocacy
– Videos can be viewed at home on the tablet device provided at no cost
– Complete questionnaires at the
– start of the study and
– after completing your adjuvant chemotherapy

ELIGIBILITY:
• 18 years or older
• Have newly diagnosed early stage cancer (stage I, II, or III)
• Planned to start or within 1 month of starting adjuvant chemotherapy treatment
• Reside on the Maui
